George Baker Jr. burned to death in his wheelchair on the patio of the CLC University nursing home in 2002. He was 54 years old.
Investigators found that Baker had been left alone on the patio to smoke cigarettes, even though he had a pressurized container of oxygen strapped to his chair.
When the tank exploded, Baker was consumed by fire before he could be rescued.
In 2004, the home was placed on the state's list of Iowa's worst care facilities. In a seven year period, while three executives were managing the chain, six of the company's seven Iowa facilities were fined $73,311 by state and federal regulators.
Last year, after the federal investigation into the nursing home abuse, each of the homes changed its name and was placed under new management.
